1. The Town of Victoria Park requires all incorporated community groups and clubs utilising Town
facilities to hold a current public liability insurance policy to the value of $10,000,000.
Please attach a copy of the Certificate of Currency to this application form.
2. Submission of this application does not guarantee allocation of your requested grounds. If
this application is accepted by Town of Victoria Park, then it remains conditional and does not
form an agreement until a Seasonal Reserve Hire Agreement and Conditions document has been
completed and signed by both parties.

Alcohol consumption

Consumption of alcohol is to be conducted in an orderly manner and comply with the
requirements of Town of Victoria Park and the Department of Local Government, Industry Regulation and Safety (Racing, Gaming and Liquor).

I understand that a seasonal booking will not be confirmed until a Seasonal Hire Agreement has been completed and signed, and a copy of the club’s public liability insurance has
been submitted.
